During your visit to Lakewood Dental Smile, we want you to be able to focus on your smile and not on your paperwork. We have provided access to our patient forms for you to complete prior to your dental appointment in Dearborn, Michigan. If you prefer, you can print out your forms and bring them with you to your visit with our dentists. You can also complete and submit your paperwork to our office electronically.
